CRC provides caring, competent and comprehensive mental health services for children, families and young adults up to age 24.
When you call us at 419-352-7588, our experienced Intake Coordinator will start to gather information about your family’s needs. This conversation gives us a general idea of your concerns and typically leads to an appointment for a Mental Health Assessment (MHA). During this call, please let the Intake Coordinator know if you would like more information about a particular program or group.
The MHA allows us to learn more about the nature of your child’s needs and helps us to determine the appropriate treatment services to recommend. A highly qualified clinician will complete a family interview, administer questionnaires about child and family functioning, and discuss treatment options that will best meet your child’s needs.
CRC accepts Medicaid and most health insurance plans. Funding through the Wood County Alcohol, Drug Addiction, and Mental Health Services (ADAMHS) Board Levy allows us to offer services to Wood County residents under a generous sliding fee scale based on income and household size.

ASSESSMENT PAPERWORK
- Download the forms on your computer and fill them out electronically. Save completed forms to your desktop, then email completed forms to crckids@crcwoodcounty.org.
- Download the forms on your computer, print them, and fill them out on paper. Keep these forms and mail them back in the envelope that will be sent to you with forms you will complete on the phone the day of your appointment.
- If you do not have a computer, or if you prefer to complete them by phone, please contact CRC at 419-352-7588 and someone will complete the questions with you by phone.
