Home-Based Services

In response to the key philosophy that mental health needs cannot be isolated or treated apart from other needs or problems of a child, our home-based services are designed to assist youth in gaining access to necessary services to address the wide range of health, social and psychological problems.

Case Management
CRC Case Managers assist and support youth in gaining access to services that are essential in meeting basic human needs. This may include coordinating care with family members, agencies, and other individuals to ensure medical, social, and educational services are provided. Case Managers advocate for youth and work to remove barriers for a healthy and successful life. 

Youth and Young Adult in Transition (YYAT)
A program that works with youth and young adults ages 16-24 to identify and develop a plan to achieve goals using their strengths, skills, and support systems. These services aim to improve positive relationships, develop strategies to cope with stress, build independent living and job skills, increase money management, and provide support in reaching educational goals.

We are not currently taking referrals for YYAT at this time.

Functional Family Therapy
An evidence-based, short-term treatment for adolescents ages 11-18 that has had great success with preserving families in need by providing clear strategies for behavior change. It engages all family members in changing the patters of how they communicate, problem-solve, and get needs met, helping to create long-lasting change in a youth’s problem behaviors.

Intensive Home-Based Therapy 
An intensive therapy that Includes in-home and community support for parents/guardians and youth. These services provide education on child development and support for healthy child and family functioning.

Please note, while we don’t accept direct referrals from parents/guardians for specific programs, every child will receive a comprehensive mental health assessment prior to receiving services. This process is designed to develop a diagnostic forumulation and identify the child’s relevant needs. The mental health assessment forms the basis for all subsequent treatment planning.
